Or you’ve been diagnosed with type 2 diabetes, pre-diabetes, insulin resistance, PCOS, or hypoglycemia and would like to use a CGM to understand your body’s response to the food you’re eating.

You don’t care about subscribing to fancy apps like Levels or Nutrisense when the Freestyle Libre 3 and Dexcom G7 already come with their glucose monitoring apps that work just fine.
You prefer to self-monitor independently without continued coaching.
You will need:
Bloodwork within the last 12 months showing HbA1c or fasting glucose levels if you’ve been diagnosed with diabetes or pre-diabetes.

Photo of your driver’s license/ID card - front only.
Photos of your health insurance card - front and back.
Your pharmacy information.
Your list of meds and supplements.
