And/or you need a refill on your asthma medications.
You’re not severely short of breath or in respiratory distress.

Photo of your driver’s license/ID card - front only.
Photos of your health insurance card - front and back.
Your pharmacy information.
Your list of meds and supplements.
Note: If you have new wheezing or shortness of breath without a previous diagnosis of asthma or other lung issues, you must be evaluated at your local doctor’s office or urgent care. If you’re severely short of breath, call 911.

Please note that Quick Care services are for uncomplicated, simple, acute conditions and mild to moderate flare-ups of chronic conditions. “Acute” means the illness or condition is expected to be temporary or self-limited with treatment and/or supportive measures.
The cost of medications is determined by your pharmacy.
Quick care does not include the advanced testing and Precision Health services rendered through the Health Edits. Please schedule a separate Health Edit if you would like a comprehensive health evaluation and management of chronic medical conditions and proactive care management.
